The Surescripts Medication History services provide prescribers with the electronic delivery of PBM/payer claim data and/or pharmacy fill data.

Medication History for Populations (Panel) is designed to assist authorized health systems, hospitals, risk-bearing groups and their authorized agents to electronically access a patient’s medication history, including but not limited to data sourced from PBM/payers and retail pharmacies, while only used in connection and conjunction with the treatment of a specific patient and for the purposes of better managing the health of patient populations.

Medication History for Populations (Prescription Notifications) is designed to assist authorized health systems, hospitals, risk-bearing groups and their authorized agents to electronically access a patient’s medication history notifications based on pharmacy fill data, while only used in connection and conjunction with the treatment of a specific patient and for the purposes of better managing the health of patient populations.

Surescripts created this document to assist customers in enrolling patients for Medication History for Populations (Panel) and Medication History for Populations (Prescription Notifications). The audience for this document includes any customer responsible for developing a system interface for these electronic messages.
