users can create prior authorization notifications after signing into the benefit plan initiated prior authorization portal building a new notification requires the input of the prescriber, patient with confirmed benefits, and prescription information prescriber search the prescriber search requires an npi to be entered users can access the nppes npi registry https //npiregistry cms hhs gov/ and search by prescriber name to find the npi if one is not provided or available enter the npi and click search for prescriber the prescriber location with correct fax number is then selected note the fax number of the location selected will be used to deliver the prior authorization with the associated trx code if a location with an appropriate fax number is not listed, a new location can be added by clicking on the + add new prescriber location enter the required information to create the new location and click add prescriber location to continue patient search full demographic patient information is required to identify benefit coverage enter the required data fields and click search for patient coverage information for the patient will be returned select the plan / pbm where the prior authorization will be processed and click confirm patient and coverage note the benefit coverage information returned will be limited to the user associated organization if a patient has dual coverage, a list of each individual plan information will be displayed for selection prescription information the medication being prescribed is required medication entry uses a search field to select the correct medication description click confirm medication after selecting the medication being prescribed pharmacy information the dispensing pharmacy of the prescription can be included if needed this entry is optional and can be bypassed by clicking skip when including a pharmacy, the name, city, and state are required to search for a pharmacy to select after selecting the pharmacy, click confirm pharmacy to continue prior authorization notification review users will be brought to a final review page after adding the prescriber, patient, and prescription information of the prior authorization notification the priority level of the prior authorization will default to standard and can be updated to expedited as needed select send pa notification to continue prior authorization notification sent users will be presented with a summary of the prior authorization notification and the associated trx code this trx code with the prior authorization details will be faxed to the assigned prescriber location fax number users can instruct the prescriber or delegate to use the fax information to complete the prior authorization or can provide them with the trx code and direct them to the prior authorization gateway at http //www surescripts com/priorauthportal http //www surescripts com/priorauthportal note provider users need to create an account in the prior authorization gateway before they can use the trx code to complete the prior authorization prior authorization fax notification fax template example
