product overview note the record locator & exchange product is based on the integrating the healthcare enterprise technical framework surescripts rle allows requesting locations to electronically discover potential locations of patient records within their source hospital or vendor systems and provides support to retrieve those records using integrating the healthcare enterprise (ihe) standard transactions the surescripts implementation of rle provides requesting locations access to the following sources of information surescripts produces an on demand document known as the surescripts location summary it is formatted following the health level 7 (hl7) standard of a clinical document architecture (cda) this document may be retrieved using standard ihe transactions sent to rle using network transactions to identify healthcare locations where patients have previously received care, rle will provide the requesting healthcare provider with a cda containing the names and contact information of the providers/locations where the patient has been seen this information may then be used to contact those locations to obtain the patient’s records when queried, rle returns information required to retrieve patient documents from other rle enabled vendor systems option 1 – records may be requested through rle exchange will be facilitated by rle using record locator transactions to query and retrieve clinical documents from discovered responding locations on behalf of the requesting location option 2 – rle provides endpoint information (home community id) for other ihe responding locations the requesting location will use existing, peer to peer connections to query and retrieve directly from the ihe enabled responding location responding locations will provide access to patient rosters by sending one of the following (1) an admit, discharge, and transfer (adt) data feed or, (2) means to accept cross community patient discovery (xcpd) queries from surescripts responding locations also have the option to enroll patients for rle services or patient notifications via an adt message and then send patient activity updates via an adt data feed for more information, see the record locator & exchange adt implementation guide responding locations will receive requests for records from surescripts on behalf of an rle ihe enabled requester ihe standard transactions will be sent by surescripts to query and retrieve patient clinical documents from the responding location responding locations may also receive ihe standard transactions directly from requesting locations to query and retrieve patient clinical documents, if preexisting peer to peer connections are established about this guide this guide was created to assist vendors in developing and transferring messages needed to electronically query locations of patient records from the surescripts rle and identify patient records within their source healthcare facilities or vendor systems and facilitate the exchange of records found the audience for this document includes any customer responsible for developing a system interface for these rle messages this guide is meant to support, integrate and further clarify the ihe and hl7 guides and best practices as used when transacting through the surescripts network it does not reproduce the base standards in their entirety when developing, this guide should be used along with additional documentation referenced and applicable customary coding standards document references please reference the following documents when reading this guide surescripts provided materials record locator & exchange adt implementation guide connectivity and authentication implementation guide directories implementation guide additional required external materials to be obtained by the customer note these external materials contain additional obligations that must be met by the customer in order to access record locator & exchange carequality interoperability framework documentation https //carequality org/resources/ https //carequality org/resources/ carequality connection (cc) terms (latest version) this document contains the standard terms that customers are required to make legally binding on their members, customers, or participants who engage in exchange activities through the carequality framework and are listed in the carequality directory query based document exchange implementation guide (latest version) the initial carequality use case is query based document exchange hl7 implementation guide for cda® release 2 integrating the healthcare enterprise it infrastructure technical framework https //www ihe net/resources/technical frameworks/
