rxtransfer overview the rxtransfer message is used in both the push and pull scenarios it allows the source pharmacy to respond to the rxtransferinitiationrequest with an approved or denied response it is also used by the source pharmacy to send an unsolicited (push) transaction to the destination pharmacy at the patient's request rxtransfer requirements send separate push messages for each prescription that needs to be transferred an rxtransfer response of denied applies to all prescriptions requested and must contain a reasoncode pharmacies are required to be enabled for all three transactions (rxtransferinitiationrequest, rxtransfer, and rxtransferconfirm) the source pharmacy is required in the medication transferred > medication dispensed element if transferring a controlled substance in the medicationtransferred element, the prescriptionpreviouslyfilled is required and designates if a prescription has had any dispensings if there is a known dispensing, then medicationdispensed is required to be included rxtransfer epcs requirements controlled substances can only be transferred if they were originally prescribed electronically the medicationprescribed, medicationdispensed, pharmacy and pharmacist include elements required to be captured on controlled substance transfers the digital signature must be transferred if it was included in the original prescription all fills should be documented in the medicationtransferred/medicationdispensed composite rxtransfer best practices rxtransfer messages should be treated as a high priority and response times should adhere to all state/federal regulations if request is no longer necessary, then the retraction/withdrawal message should be used the pharmacy should review all rxtransfer messages they should not be deleted from a processing queue without a response being generated to the source pharmacy if a transfer scenario is not supported, such as “all” or controlled substances, then a clear response should be provided such as “pharmacy location does not support the rxtransfer workflow for “all” the pharmacist element should be used to identify the pharmacist responding with the transferred prescription(s) if allergies, observations, supervisor and/or notes are included in the original prescription, it is recommended to include those in the rxtransfer message the source pharmacy should send rxfill status of transferred to notify the prescribing system that the prescription is now at a different pharmacy for additional details see rxfill/rxfillindicatorchange best practices
