the following sections provide additional information for rxrenewalrequest and rxrenewalresponse elements rxrenewalresponse table customers are required to support each rxrenewalresponse type indicated in the table below when sending an rxrenewalresponse, follow the rules below to determine the workflow and the type of response to send the requesting pharmacy the rxrenewalresponse type should be determined by the application, rather than the end user, using the criteria in the table r 303 the rxrenewalresponse type shall be determined by the rules in the following table exceptions to this requirement include items in the common message header and the optional structured and codified sig response type definition approved an approved response is appropriate when the pharmacyrequestedrefills value (if sent by the requesting pharmacy) is returned in the numberofrefills and no other changes or additions were made the numberofrefills in an rxrenewalresponse represents the total number of dispensings authorized and must be a value of 1 or greater the writtendate is updated to the date of approval when the pharmacyrequestedrefills is not present in an rxrenewalrequest, the response can be approved if no other changes or additions were made approvedwithchanges an approvedwithchanges response should only be sent when the prescriber is approving a number of refills which differs from the suggested pharmacyrequestedrefills value sent by the pharmacy in the rxrenewalrequest the writtendate is updated to the date of approval for all other changes or additions to the request, see replace denied a denied response indicates the rxrenewalrequest has not been approved by the prescriber the information in the response should match the information in the rxrenewalrequest denial reason or reason code must be included to explain the denial pending a pending response is an informational response allowing the prescriber to temporarily hold the rxrenewalrequest while sending a response to the pharmacy it is recommended to include a reason code and expected response date with the pending response replace replace a replace response should be used whenever any change or addition is made beyond the number of refills suggested by the pharmacy in the rxrenewalrequest the writtendate is updated to the date of approval all fields may be changed with the exception of the patient’s dateofbirth if an incorrect patient dateofbirth is received on the rxrenewalrequest, a denied should be sent to the pharmacy and a new prescription should be sent rxrenewal element usage in the newrx, the total number of dispensings is 1 + the refill quantity; however, in the rxrenewalresponse (including the replace type) the total number of dispensings is only the refill quantity the application should make this distinction clear to the user during the approval process r 309 an automatic response for rxrenewalrequests shall not be sent back to the pharmacy exceptions to this requirement include an automatic denial for non support of compounds or supplies, a deceased patient, or when the original prescription contained the prohibitrenewalrequest flag automatic response a response that is generated systematically without allowing the receiver to review the request numberofrefills the numberofrefills is used by the prescriber to communicate a number of dispensings for the prescription a newrx and an affirmative rxchangeresponse have a total dispensing equal to a first fill quantity plus the number of fills identified in the numberofrefills element examples newrx quantity is 30 tablets number of refills is 3 total dispensings = 4 total quantity to be dispensed = 120 tablets rxchangeresponse affirmative responses are approved, approvedwithchanges, and validated quantity is 30 tablets number of refills is 3 total dispensings = 4 total quantity to be dispensed = 120 tablets rxrenewalresponse affirmative responses are approved, approvedwithchanges, and replace it uses the numberofrefills element to communicate the total dispensings quantity is 30 tablets number of refills is 3 total dispensings = 3 total quantity to be dispensed = 90 tablets note the numberofrefills in a cancelrx does not change the intent of the message to discontinue dispensings
